I acknowledge that I have voluntarily chosen to undergo a body massage and traditional hair wash treatment provided by TR.beautyspa. I understand that these treatments involve specific techniques, products, and potential risks.

I acknowledge that TR• Beauty Spa does not accept insurance and agree to proceed with the service.

I have been informed and understand the following:

1. Body Massage:
• A description of the body massage treatment, including the techniques and areas of the body that will be addressed.
2. Traditional Hair Wash:
• An explanation of the traditional hair wash procedure, detailing the products used and the process involved.
3. Potential Risks:
• I am aware that, like any spa treatments, there may be inherent risks or side effects. These could include, but are not limited to, soreness after the massage or reactions to hair care products.
4. Product Usage:
• I have been informed about the massage oils, lotions, and hair care products that will be used during the treatments. I have disclosed any allergies or sensitivities that may affect the outcome.
5. Post-Treatment Care:
• I understand and agree to follow any post-treatment care instructions provided by the massage therapist and hair care specialist to optimize results and minimize potential side effects.
6. Clothing:
• I acknowledge that I will be appropriately draped during the massage session, and I am comfortable with the level of undress necessary for the treatment. For the hair wash, I understand that my hair may get wet, and I am prepared for this.
7. Informed Decision:
• I acknowledge that I have had the opportunity to ask questions and seek clarification regarding the body massage and traditional hair wash. My decision to undergo these treatments is voluntary.
• Consent for Touch:
• I understand that physical contact will be involved during the massage and hair wash, and I provide my consent for such contact in a professional and appropriate manner.

I understand that TR• Beauty Spa is committed to providing safe, professional, and individualized treatments using appropriate techniques and medical-grade technologies. I acknowledge that treatment outcomes vary between individuals and depend on factors beyond the control of TR• Beauty Spa, including my skin condition, medical history, lifestyle, healing response, and compliance with aftercare instructions. Therefore, no specific result or degree of improvement can be guaranteed. I understand that an outcome different from my expectations does not, by itself, indicate negligence or improper treatment. All treatments and packages are Final Sale and non-refundable, except where required by applicable law.
